You should report AI issues when:
The system causes harm or errors affecting people’s safety, privacy, or rights
The system behaves unpredictably or outside of expected norms
A near-miss occurs that could’ve caused damage
Users report repeated confusion, bias, or concern
You notice a sudden drop in performance or trust
